The sheer volume of information presented here is the strength of this comprehensive manual on lung cancer. Writing with freelancer Wernick, Henschke, chief of the division of chest imaging at New York Hospital Cornell Medical Center, provides a medical perspective on the disease, while McCarthy, founder of the Alliance for Lung Cancer Advocacy, Support and Education (ALCASE) offers support for those afflicted as well as their caregivers. From the outset, McCarthy makes clear that the tendency of family members as well as some health-care professionals to blame smokers (nonsmokers are also diagnosed with lung cancer) when they contract lung cancer not only leads to guilt, but may actually discourage some people from seeking adequate treatment. The authors describe the many forms of lung cancer and their treatments, which include surgery, radiation, chemotherapy and alternative therapies. They explain how to apply for clinical trials as well as the benefits and risks of participating. Since earlier detection improves the chances of a cure, the authors recommends the still controversial CT scans for those at high risk for lung cancer (i.e., former or current smokers). This excellent resource details how patients can learn to cope with the physical problems resulting from some treatments, how to sustain themselves emotionally while fighting the disease and ways to navigate the health-care system. Lung cancer kills more Americans each year than prostate, colon, and breast cancer combined. The five-year survival rate is an abysmally low 14 percent largely because the disease is symptomless at a stage when most curable. With new developments in CT scanning, early detection, and treatment can improve survival to 70 percent or better. Henschke (Cornell Medical Ctr.) and lung cancer patient advocate McCarthy have written a thorough guide to the basics of how lung cancer develops, risk factors, diagnosis, treatment options, and living well with lung cancer.     "Dr. Atkins' Diet Revolution", published two decades ago, sold millions of copies but was denounced by medical authorities for its unsound high-calorie, low-carbohydrate regimen. Now it's back, slightly modified, and billed even more contrariwise as a high-fat, low-carbohydrate diet. Atkins blames carbohydrates for most cases of overweight—as well as for much fatigue, mental fog, high blood pressure, high cholesterol, heart disease, diabetes, and cancer. A high-fat diet, he says, is harmful only when added to a high- carbohydrate diet. At times, he backs off and hedges, admitting, for example, that refined flour and sugar, not all carbohydrates, are the culprits, and even that the desserts he promotes should be limited to special occasions. Still, heavy cream, butter, and cheese abound in his recipes; bacon and eggs are on his daily breakfast menu; and his program, especially the 14-day "induction diet" designed to induce ketosis, or fat-burning, turns all prevailing guidelines upside down. It will be interesting to see how this book does now that low-fat, high-carbohydrate eating has been so widely accepted by professionals and public alike. Atkins claims success with his 25,000 overweight patients (who take an average of 30 nutritional pills a day along with the diet), and he scores a point or two against pro-establishment preconceptions among researchers, but he certainly doesn't prove that his is the healthier diet. Still, get ready for a blitz.

Kowalski recommends ways to monitor blood pressure, combat the metabolic syndrome that leads to heart disease and diabetes (abdominal obesity, high cholesterol and triglycerides, insulin resistance, inflammation, etc.) and raise levels of HDL (good) cholesterol. He provides basic diet and exercise guidelines, details the components of proper electrolyte balance (sodium, potassium, calcium and magnesium) and describes potent natural remedies, including arginine, pycnogenol and grapeseed extract. There are sections on children's health and risks and a review of current prescription drugs for those who must take them. Kowalski's use of clichés and case studies bogs down the text somewhat. Still, readers facing poor prognoses will gain a hopeful perspective on the future from the author's experiences. (May)

While admitting heredity and race play a role, Kowalski asserts that these are only warnings that can be negated by good health habits. He explains the new definitions and classification of blood pressure levels, including prehypertension, and provides rationale for the updated guidelines. Risk factors for special populations of children, African Americans, women, older people, and diabetics receive brief attention. Basic information on weight reduction, physical activity, stress reduction, alcohol use, and diet as they affect hypertension is given. Some alternative medicine therapies are mentioned. Prescription drugs are briefly covered; some recipes are included. Kowalski touts five secret weapons for a blood pressure cure: arginine, grape seed extract, tomato extract, Pycnogenol, and cocoa. He cites small clinical studies to back his assertions; unfortunately, larger studies show unclear results. Recommendations for lifestyle changes are vague.     Requirements for treatment planning in the mental health hand addictions fields are long standing and embedded in the treatment system. However, most clinicians find it a challenge to develop an effective, person-centered treatment plan. Such a plan is required for reimbursement, regulatory, accreditation and managed care purposes. Without a thoughtful assessment and well-written plan, programs and private clinicians are subject to financial penalties, poor licensing/accreditation reviews, less than stellar audits, etc. In addition, research is beginning to demonstrate that a well-developed person-centered care plan can lead to better outcomes for persons served.

    Description: This book describes an individualized approach to addiction recovery and treatment and includes unique appendixes that outline actual treatment plans.
    Purpose: The purpose is to have treatment providers recognize the utility of tailoring treatment and not necessarily use an algorithmic approach. It then outlines some methods of customizing the treatment to meet individual patient needs in a biopsychosocial fashion.
    Audience: There is a very small and specific audience for this book. Mental health professionals who are focused on addiction recovery, and have the necessary resources to carry out the treatment plans, may benefit.
    Features: The book starts by arguing the importance of person-centered care while defining the term. It then delineates specific methods for individualizing care plans. It is inclusive of some of the basic tenets of addiction recovery, including assessing stages of change and having patients perform narrative summaries. The appendixes are very useful, outlining care plans in a very specific manner that I would find applicable to clinical care.
    Assessment: I did not find this book particularly engaging. The authors are not very succinct in describing basic addiction recovery principles and since the audience is likely to be versed in these, they likely will find the book lacking. Although the title seems clearly stated, the overall thrust of the book is somewhat convoluted and ethereal. The appendixes, however, are very useful. These sorts of concrete examples are what clinicians need to practice with this population. In fact, I would consider using these appendixes as a template for recovery planning. Perhaps, a sort of workbook would be more useful since the quality of the text does not match the utility of the appendixes and the audience is most likely familiar with most of what the text covers.

As an increasing number of Americans are overfed and undernourished, Pollan makes a strong argument for serious reconsideration of our eating habits and casts a suspicious eye on the food industry and its more pernicious and misleading practices. Listeners will undoubtedly find themselves reconsidering their own eating habits. Scott Brick, who narrated Pollan's The Omnivore's Dilemma, carries forward the same tone and consistency, thus creating a narrative continuity between the two books. Brick renders the text with an expert's skill, delivering well-timed pauses and accurate emphasis. He executes Pollan's asides and sarcasm with an uncanny ability that makes listening infinitely better than reading. So compelling is his tone, listeners may have trouble discerning whether Brick's conviction or talent drives his powerful performance. Particularly innovative and delicious are the dessert and ice cream recipes. Generally, Kenney's choices will appeal to both the non-raw-food-eating cook and the raw-food devotee alike with his healthful and tempting recipes. This chronicle of Al's injury, treatment and rehabilitation shows how perplexing and stressful traumatic brain injury can be for both victim and family. Crimmins (When My Parents Were My Age, They Were Old and Newt Gingrich's Bedtime Stories for Orphans) knows how to tell a story for maximum effect, filling this account with funny and outrageous anecdotes, raw emotion and predictable rage toward HMOs that won't fund optimal treatment. Like many TBI patients, Al became bizarrely uninhibited; Crimmins describes how he swears profusely and masturbates in public, and her worries about suddenly being married to a stranger: "I once had a husband who was doing a dissertation on Samuel Beckett, who had a thing for obscure Japanese cinema.... I can't imagine being married to a man who won't be able to discuss books or go to the theater with me." Despite Alan's extraordinarily good recovery, Crimmins muses, "I miss his dark side.... Now I wince as he chortles over mediocre cartoons... with TBI he has become what he wasn't before, a regular, uncomplicated guy." Though this story is an eye-opener on some levels, it remains essentially shallow. More information on neurological research would have been welcome, and attention to the experience of other TBI families (to which Crimmins devotes only three paragraphs) would have added the perspective that this self-centered account lacks.
